A Life Care Plan is "a dynamic document that is developed based on comprehensive assessment, data analysis, and research to provide an organized and concise plan for the present and projected future needs and resources required by an individual, or groups of individuals, who have experienced a serious illness or injury resulting in physiological, psychosocial, cognitive, behavioral, or emotional disabilities." (Combined definition of the University of Fla. and Intelicus Annual Life Care Planning and the American Academy of Nurse Life Care Planners, presented at the Forensic Section Meeting, NARPPS Annual Conference, Colorado Springs, and agreed upon 4-3-98).
Life care plans are developed using a "wellness and complication prevention" methodology. Review of medical records and other documentation, along with evaluee interviews, home assessments, and research of available services for the evaluee's needs are all integral to providing the most comprehensive life care plan. This can be used for future care planning of a loved-one or setting reserves for catastrophic claims as well as in the litigation process.
